OUR MISSION
The High-Fidelity Wraparound (HFW) team works collaboratively to assist families in identifying needs, navigating systems, and building on natural supports. The HFW team helps each youth 
and family to address current needs while building the skills to maintain long term success
OUR APPROACH
The HFW team utilized a Facilitator, Family Support Partner, and support Partner to allow all members of the families with the assistance of the HFW team, the family created and implements one plan to address their needs and incorporate involved.

WHO WE SERVE
The High-Fidelity Wraparound team work with youth under the age of 21 who experience mental and behavioral health challenges. High Fidelity Wraparound is available to youth and their families in Gaston and Lincoln Counties (expanding to other counties throughout the NC area).

- Lived experience as a Caregiver of a youth with SED, MH/SA, DJJ/DSS
- Advocates for Family/ Caregiver voice
- Teaches skills
- Certified in Triple P
- Assists with interventions/ CFTM follow up

Youth Support Partner
- Lived experience as a youth with SED, MH/SA, DJJ/DSS
