Our Tailored Care Management program connects you with:
- A Dedicated Care Team: With a Care Manager and access to a Peer Support, also known as an Extender, your team collaborates to support every aspect of your life.
- Individualized Care Planning: We work with you and your natural supports to build a care plan that reflects your goals, whether that includes behavioral health support, physical healthcare coordination, or access to vital community resources.
- Coordination Across Services: We link you to services within Support, Inc., including outpatient therapy, medication management, enhanced community services, residential support, and external providers when needed.
- Support Beyond Healthcare: Our team helps address housing needs, food access, employment support, transportation, and other social drivers of health that impact your well-being.
- Consistent Communication & Monitoring: We maintain regular contact with you and your providers to monitor progress, resolve barriers, and adjust your care plan as your needs evolve.
Who Is Eligible?
Everyone enrolled in a North Carolina Medicaid Tailored Plan is eligible for Tailored Care Management, at no cost. There’s no application process, and services are available regardless of age, diagnosis, or level of care.
